I wonder if something is going wrong very early in the initialisation process. Since the shell seems not to be displaying anything of use, you could try building ipxe.efi with DEBUG=efi_init:

Code:
make bin-x86_64-efi/ipxe.efi DEBUG=efi_init

That should give some debugging output pretty much immediately upon loading ipxe.efi.
